Try Gemini from Iceni http://www.iceni.com
I tried the downloadable evals of Gemini and Jade. Both work well, but Gemini needs less manual intervention before the export.

I went to this site and they offer a demo of Gemini, but it states
that it requires Adobe Acrobat Exchange v3.0. The Adobe web site
doesn't list any product like this. Could you supply some detail?

Both Jade and Gemini are plug-ins for the full version of Acrobat - the one that
can create PDF files. They don't work with the Acrobat reader.
I used eval copies of both with Acrobat 4.0. They both worked well with this
product. I preferred the functionality of Gemini. I have not used either of them
with Acrobat Exchange 3.0.
I have asked my company to buy in a copy of Gemini for me.

I have Acrobat 4.0 (not the Reader) and nowhere in the
documentation or on the box does it say "Exchange".
It does say "the essential tool for universal document exchange."
When Gemini says "Exchange", do they just mean the $200+ version
of Acrobat?

In a word - yes.
Exchange was the "Full" $200+ version of Acrobat 3.0
In Acrobat 4.0 the full version is called Acrobat 4.0, and the read-only version (not
usable to convert files with Gemini) is Acrobat 4.0 Reader.
