generated items "die"

When I move some framedocs around I often experience that generated items
like the TOC and Index kind of "die" in the book file. For some reason they
disappear from the list over items which are to be updated in the book
file - even though they still are included in the book file. Only I cannot
update them anymore, Can I somehow "revitalize" these files again and make
them ready for update using the book files update system?

Have you checked that the TOC and Index files haven't somehow become
read-only or locked? If that is the
case then that would explain why they appear in the book but don't get
updated when you update the book.
