Now if you have a server which doesn't have a graphics card of keyboard
attached, but you have a VT terminal attached, how can you achieve the same
function, i.e. booting from cdrom and getting into a mini-root to enable
file system changes say???

Not sure what the last part of your sentence means, but I think
you want to boot into single-user (non-GUI) mode. Use:
boot cdrom -s
at the ok prompt.

You do exactly the same thing. Just on a serial console, issue a break,
'boot cdrom', then do what you need to do.
You might have to ensure that the CDROM drive is aliased correctly,
usually the system expects it on SCSI ID 6 or master on the 2nd IDE
channel.

I've tried booting from the cdrom using the VT term and it just goes
straight into the install program, i can't see any options to allow me to
break out and get to a shell.....

boot cdrom -s
will give you a single user shell that you can mount the internal drive with and work on it.
