Can't Create New Blank Disk Image

I'm no longer able to create a new blank disk image or sparsebundle
using disk utility. I get the error message "Operation Failed with
status 13" or "Operation Failed” with status -5000" trying to create an
encrypted image.

Also, when I mount an existing encrypted sparsebundle, it no longer
appears on the Desktop, though still appears in the Finder window under
Devices.

I reinstalled the OS but that didn't solve the problems.

21.5" late 2012 iMac, El Cap 10.11.5, hundreds of GB of HD free space,
no other problems.

What's going on?

At exactly what point does the error occur?

Does it happen after you fill in the information in the new disk image
dialog (document name, location, size, format, and so on) and click the
Save button?

If so, what happens if you try to create the disk image saving it on
another storage volume?

Yes, it happens right after I click Save.

And I was able to create an image on an external USB hard drive.

I wonder if the ownership and permissions of the directory you are
attempting to save to when the problem occurs are set incorrectly.

Good call!

I cleaned up the ownership and permissions and can now create new
unencrypted and encrypted images, the latter appearing on the desktop
when mounted.

But the (only) existing encrypted sparsebundle still doesn't show on the
desktop when mounted- though it used to. If I can't get it to show, I
guess I could create a new one and copy all the files from it to the new
one then delete.

I don't know where to look for /Volumes and can't find it.

The existing sparsebundle shows both in Disk Utility and in the Finder
in the Documents directory which is where I have been keeping it.

Cmd-Shift-G in the Finder. Type "/Volumes". Hit <Return>

You have to select "Go to Folder..." from the Go menu in the Finder and
type in "/Volumes". You can also start Terminal and enter "cd /Volumes"
and then "ls".

If it's in /Volumes, then the disk image is indeed mounted. I'm not sure
why it wouldn't show up in the Finder though.

Are you sure you have the Finder preferences set to display disks on the
desktop?

Does the mounted image appear in Finder window side bars where other disks
are listed?

But they don't. Disk images opened (and mounted) in Disk Utility do not
appear on the dekstop or in sidebars and do not appear in Finder (or
Path Finder) windows unless "Go to Folder..." is used. Weird, innit?

It doesn't make any difference. Images mounted by DiskImageMounter (with
a double-click in Finder) appear as volumes in all the usual places.
Images mounted by Disk Utility (using "Open Disk Image..." in its File
menu) do not.

That's the way my system is behaving. Yours is apparently doing things
differently.

It does for images opened from the Finder.

Where did Wade say he was mounting the sparse bundle image with Disk
Utility rather than simply opening it in Finder?

It's not clear how Wade is mounting this image. My assumption has been that
he does it the typical way: by double-clicking the disk image file icon in
a Finder window.

It does appear in the Finder bar and I cna access the directories and
files on it. But it does not appear on the Desktop.

Other disks appear on the Desktop as they always have as does a newly
created encrypted sparsebundle. And up until a week or two ago, so did
that old encrypted sparsebundle when it was mounted.

Everything is ticked to show- and everything does, except that old
encrypted sparsebundle.

Hmm...

In case you haven't already, you might try:

Restarting the Dock, which is responsible for portions of the desktop.
You can quit it in Activity Monitor. It will relaunch automatically.

Restarting the Finder by Option-double-clicking the Finder icon in the
Dock and choosing Relaunch from the contextual menu.

Logging out and back into your Mac OS X account.

Restarting the computer.

Tried each of those-- no joy.

Thanks for all the detailed help on this. I'm really happy that the new
disk creation function is restored.

As for trying any further to get the old encrypted sparsebundle to show
on the Desktop when mounted? It's time to quit

As that sage philosopher Kenny Rogers said, "You've got to know when to
hold 'em, know when to fold 'em, know when to walk away, and know when
to run".

I'll make a couple of backups of the data on that sparsebundle, create a
new one, copy the data over, then delete the problem child!

The only other thing that occurs to me to try is:

Mount the disk image, view the /Volumes folder, do a File > Get Info on the
mounted image, highlight the icon in the Info window, and press the delete
key on the keyboard. That will remove any potential custom icon, on the off
chance that a corrupt (invisible) icon has somehow been assigned to the
volume.

I encountered this error when trying to make a container for 5MB.

I figured size might be an issue so I tried again at 10MB and was successful.

Just happened right now.
