Whose 386 SBC is this?

I have what looks like a single board 386 computer with the following
- plugs into an AT-style bus
has space for 8 SIMMs
- has a connector for the keyboard on the bottom near the bus fingers
- has a push button switch through the back bracket
- has a 1/8" phono jack thru the back bracket
- a 4 switch dip switch at the top back of the card
- 386DX-16 chip
- two 40 pin headers next to the SIMM sockets
- 572-A-0301 screened on the front edge
- a sticker with 572-DGA-301-202
-001 REV H 8918
386/16 10171 on the back of the card
- a sticker with 572-I-900
B386 BIOS
V1.1009 on the BIOS? chip
NO manufacturer listed but it uses the CHIPS chipset.
Any help appreciated!!!
