I finally got my 4th star just as the 6th year began. I also survived
a bomb threat the day before. Is it possible that it needs to be
after a certain year to get the star, or after surviving the bomb
threat?

MBA games -- Mindless, But Addictive), there're some kludges noticed:
Restraunts People only eat LUNCH! They don't eat dinner.
Towers Build Wide, not tall! Use stairs, not elevators.
The system expects things working left to right
ie, you put a small section on the extreem left,
and another on the extreem right, it will make
the entire void area into unused floor space.
Stairs Put the office blocks in the lower floors and make them
use the stairs. One set of stairs can sustain the entire
building's people!
Stress People get stressed over WAITING for the elevator, if there
is no elevator service (they have to take the stairs), there's
no waiting, and no stress!
Elevators People are dumb -- they will wait for an elevator and start
glowing red. If a long queue forms for a single elevator
(somehow, those lemmings aren't smart enough to use the stairs
or another elevator), disable the elevator from stopping at
that particular floor -- everybody will disappear and become
unstressed.
Condos Run down condos that people have moved out of -- say, if you
originally sold the condo for $150k, and now it's sold back to
you in terrible condition, drop the price to $40k and the
terrible condition goes away, then increase the price again
to $200k, and people will buy it (in excellent condition),
$50k profit, too!

Keep an eye on the info bar (where your current star rating and cash
balance are) and see what messages pop up. NOTE: the Sims cannot take
more then two elevators per trip. Perhaps some are unable to get from
their offices to a medical center in only two trips? This happened to
me so I built medical centers every 15 floors near the lobby and sky
lobbys so every sim was within ONE elevator trip of a medical center.
