There are no hard and fast rules as both pen'n'paper and CRPGs have
blurred the definitions so much from their origins with different
publishers using similar terms in different ways.
If you use the story analogy and treat a campaign as a story arc then
there is no reason for it not to continue across several sequels,
consider the Hobbit's Ring Quest (campaign) in Lord of the Rings which
covered most of 3 books (if each book was a discreet game then the
campaign has crossed games and is still the same campaign, the Ring
Quest).

I don't have a solution but I admire the problem.
Not necessarily. Some name themselves after the game world area they
take place in but are still directly tied into the previous game's
campaign story and characters.
