Swim through the pool under the waterfall until you come up in a small
cave. There should be a raptor running around nearby. There is a place in
the wall next to the pool where you can climb up. Climb up there and
eventually you'll come to the cog.

1st cog is after you kill the T-Rex, run to the far side of the valley.
Walk into the waterfall in the center of the far wall, and siwm through
to the next cavern. (When you jump out of the water BE PREPARED! There
is a Raptor here). After killing the Raptor, climb the blocks to the
top. This is the first of three.
2nd cog is go past the dead Raptor and back into the valley. RUN with
guns out (yes, TWO more Raptors here!) Go to the yellowish building.
Dive into the water there and go to the right and down.
3rd cog is found after exiting the yellowish building. Run to the right
side and climb up the blocks. There is a path to the broken bridge
here. Cross the bridge (running jump, grab)
After finding all 3, go back to the beginning of Lost Valley and go up
the rocks behind the waterfall. Go to the left from the starting point
and jump over the stream 3 times (carefully - I'd suggest saving your
game often). At the 4th ledge, do a standing jump and grab. Go through
the passageway until you come to a rope bridge. This will take you to
the machine where you have to put your cogs.

Go to the temple like building at the far end of the valley. The third
cog is in the pool, I believe on the right side.
You have to jump across the broken bridge to get the third cog in the lost
valley (do a running jump it can be done).

I'm not sure if this is the cog you need, but it's the one that literally
took me hours of mind numbing searching to find (and was one of the key
factors in my decision to return this game) anyway, if I remember
correctly, after diving into a pool in the temple(or by the walls of the
valley), you can swim underground a bit and re-emerge in an area that is
mostly black/dark, and has a raptor waiting for you right around a close
corner (bear with me folks, it's been a while) anyway, in that area,
right above that pool even I think, there is a ledge that is hardly
visible, and up there should be your cog. I hope this helps

This question seems to me to be The Most FAQ about TR, and all the replies
to it, such as this one, always give the same advice - under the waterfall,
swim a bit, come up by a raptor, and don't mention getting chewed a bit by
said raptor before proceeding. I'd found though that you can approach this
area from the other direction by exploring the openings in the cliff nearer
the entrance to the Lost Valley - you can get to a position by following
the tunnel from one of these openings where you can shoot the monster from
a high position completely safely. Possibly doesn't get the blood racing
as much as going hand-to-hand with the raptor admittedly, but each to their
own!
