I expect they put that in the game just to tease you and make you
wonder what and where the biggest monkey head was.
It's kind of like the mystery of Shakespeare's second best bed.
In his will, Shakespeare left his second best bed to his wife,
but no one really knows what his first best bed was. It's a
mystery for the ages, as is perhaps the biggest monkey head.

I always thought it was just a throw away line, much like Simon the
Sorcerer when he says "It's a fine example of its kind" when asked to
examine some weird looking object. I took it as verbal humour only,
not anything related to the games.

FWIW, this was a standard joke on the old tv series Get Smart (a secret agent
parody if you're not familiar with it). When something with an unusual or
exaggerated aspect would show up, the lead character would often say 'That's
the second *-est * I've seen!".

This is an old joke, if anyone played Commander Keen 4, the Dopefish
