Yeah! If memory serves me correctly, go to "Options" and hit "enable
mouselook" or something like that.

If that fails you, at the console type "+ mlook". That should do the trick.

BTW, both of these are in the manual, I believe.

Does anyone know of a way to enable a constant mouselook in Hexen II? As it
is, to aim in a vertical direction I have to use the keyboard which is
complete crap. I can only change my aim in large increments and it is too
slow. By the time I aim I have been shot a couple times and my target is
already moving again. I want to be able to use the mouse to control my aim
both horizontally and vertically, like in Jedi Knight and Unreal, without
having to find a key to hold down while I am doing it.

I put +mlook in the autoexec.cfg file.

I poked around and figured it out. You have to go to the command line and
type "+mlook". This was not documented anywhere I could find, but I was able
to get the list (without explanations, by the way) of commands by typing
"commands" at the command prompt. This created a txt file called
commands.txt which lists them all. I don't know if this works with versions
other than 1.11 (the latest patch), but now everything is smooth. I'm a
happy Hexen II player.
