Hexen: Dungeon & Axe keys?

They are on later levels, so you will have to come back to the gibbet to unlock
those doors. It took me some time to find the exit from that level, so let me
just say to look in the water near where you entered.

Did that, but found only deadends in effluvium. Oddly, when I map
I can see the rooms behind the doors to the west and southwest, but I
can't go through them. Only thing I can do is go back through the
teleporter. I've pressed every wall/switch/torch in the other three parts
of the hub, but all I've got is a rusted key. Same question for me, I
guess: How do I get the dungeon and axe keys, or how do I continue??
