Synopsis: Based on 9 years experience in the area of software safety
servicing the safety-critical sectors of medical devices, nuclear power,
defense, aerospace and transportation, a great deal of insight into this
area has been accumulated by these two people. Their insight is based on
post-accident safety analyses (beginning with cancer therapy machine
software), post-design safety analyses and life-cycle approaches to safe
design and safety analyses. A discussion of existing techniques and their
effectiveness and shortcomings will take place.
