Does anyone know how to attach a shaft to a typical Futaba hobby servo
motor? Is something like this sold anywhere?

Nothing like that is sold to my knowledge, however, it can be made.
Get aluminum rod to your desired diameter, get a tap that matches the
screw that holds the servo arm to the servo, tap the rod, get a long
screw that matches, snip off the end and thread into servo and rod.
This will only work with metal gear servos however as the plastic geared
ones have self-tapping screws that obviously won't stand the stress in
the plastic. If you have the shaft well supported then you might get
away with a plastic gear servo anyway...

There's a (fairly) new hookup system for hobby servos called RDS. The
connectors are being manufactured by Kimbrough. You should be able to
get them from hobby shops that stock their stuff. It uses a splined
top that holds the drive shaft. Have a look at
http://www.bmi.net/propt/rds/ for more details.
