Assuming you mean a genuine P7 mailbox - then Yes - by the protocol
you can query the mailbox without retrieving the contents. However,
the software that you are using might not support it.

Probably too complex to do pure P7 from VB, however you might be able
to interface via an intermediate API (like the UA-FI from MaxWare (or
whoever looks after it nowadays).

we don't have out own X400 server, but have a mailbox on the X400
network.
We use MaxAware AU-FI software to get the messages from this X400
mailbox. I was wondering if I could check if there is anything in the
mailbox.

To communicate to X400 Mta, you can use Mailmax UA-FI that can be
easily integrated in your application

This is indeed achievable with UA-FI as long as nothing has changed in
the program since I last used it you can use Fetchlist=2 to download
the herders only.
