According to the Netscape Handbook (Help | Handbook):
"The Flag icon shows if the message has been marked as noteworthy.
Click the icon to toggle the flag status."
As to what use making a message 'noteworthy' is? Good question. I
suppose it can be used as a placeholder.. If you find a message
interesting and want to reply to it, but would like to finish reading
the rest of the messages in the thread or group first (and there's a LOT
of messages in that thread or group), then you can mark that message
with the red flag, then once you're done simply scroll back up until you
see the easily-recognizable red flag.
Anyone else have a use for it?

They do nothing, but they are persistent. The idea is that you can
flag an article so you can quickly find it at a later date.
