Windows/286 didn't have the second mode mentioned above.

You aren't talking about Windows/286, the subject actually under
discussion here, but are instead talking about other versions of
DOS-Windows. Moreover, you are confusing DOS-Windows 3.x with
DOS-Windows 9x/ME. The '386 DPMI host in DOS-Windows 3.x is in
WIN386.EXE.

Yes. The VMM file for Win3.x is WIN386.exe.
It moved to VMM32.VXD only in Windows 95.
