ATM on NDIS 5.0

Anyone have any experience (preferably pleasant) installing ATM NICs on
NT 5 or Win 98? I've tried installing FORE's PCA200E on NT 5.0, but
keep getting an unhandled exception in ntoskrnl.exe (probably due to
PCA200E.SYS). (I could have sworn it installed once on another system,
but I never got LANE working -- so I'm at it again).

What I'm looking for is:
- an NDIS 5.0 driver for FORE's ATM NIC (or at least verification that
the one on the distribution actually works).
- any other vendor's working implementation
- configuration lessons with NDIS 5.0 and services (e.g., don't run both
ATMARP and LANE simulaneously, etc).
