I am installing WinNT workstation SP4 on a dual 800Mhz PIII system. I
can't seem to get it to consistently recognize two processors. Most of
the time it only recognizes processor 0. The bizarre thing is that
sometimes it will fully recognize both at boot up!

Here's an interesting observation under Systems Diagnostics...
When the two processors were recognized this is what I saw...

Processor list:
0: x86 Family 6 Model 8 Stepping 3 GenuineIntel ~801 Mhz
1: x86 Family 6 Model 8 Stepping 3 GenuineIntel ~801 Mhz

When only one processor was recognized and working this is what I
saw...

Processor list:
0: x86 Family 6 Model 8 Stepping 3 GenuineIntel ~801 Mhz
1: x86 Family 6 Model 8 Stepping 3 ~801 Mhz

It looks like only the descriptions are different. Does anyone know
why this might happen? Is it a sick processor? Why did it work
before?

Another note: when I switch around the processors into different slots,
the system won't boot at all.

Then you have an intermittently bad second processor. Take it (or both)
back to wherever you got the processors, tell them what happened, and get at
least the one replaced. However, make sure the replacement is the same
stepping as the other ("x86 Family 6 Model 8 Stepping 3").

Dual-Processor-Systems are highly sensitive, as far as i have
experienced.
Best is, to use two processors out of the same charge. To do so, compare
the
serial-numbers. They should differ only in the last few numbers.

And: don´t use Servic Pack 4, i do not have the best experience with it.
Use SP5 or 6 instead.
