How to make a desktop icon . .

What's the proper way to create your own desktop icons ?
What format ?
What size ?

I can't for the life of me find the right format to save a graphic to
so that it is acceptable to NT as a desktop icon.

I'm using photoshop, saving as a bitmap, 64 x 64 pixels, rgb or
indexed color, then renaming the file to *.ico.

But when I try to use set the new icon from the "properties, change
icon..." dialog, I get an error message that there is no icon in the
file.
