Dell 4100 can't boot - HD invisible?

We have a Dell 4100 (100MHz 486, builtin NCR SCSI)
with a 1GB DEC hard drive and Toshiba CD-ROM (both
off the built-in SCSI). We bought the 2.0 FreeBSD CD-ROM,
made the floppies, started the install, and can't get
past the fdisk (why does this feel like a games spoiler
question?)

unix is seeing the CD-ROM fine, but gives no indication it
sees the hard disk. I try fdisk, and it shows NO drives
available! It just laughs at me if I try to tell it to
use drive 0 anyway. I've played around with /kernel -c,
disabling everything I can think of - no help.

Any suggestions?
