I would like a reminder that only requires me feed in a name and birth
date. Then have a reminder pop up a few days before.

I just put each person's birthday into my contacts, and iOS / Mac OS
automatically reminds me when their birthday arrives. Easy!

First, configure iOS to alert you on birthdays:

1. Open the Settings app.
2. Tap "Mail, Contacts, Calendars".
3. In the "Calendar" section, tap "Default Alert Times".
4. Tap "Birthdays".
5. Choose when you want to receive your alerts.

Next, add a birthdate:

1. Open the "Contacts" app.
2. Scroll to a contact and tap "Edit".
3. Scroll to the very bottom and tap "Add Field".
4. Tap "Birthday".
5. Add the birth date to the contact.

Now iOS will remind you when that birthday arrives.

Why bother when iOS does it already?

Thank you very much for the "how to" just what I wanted.

Best regards

Will this work under OS 10.8.3 and on an iMac?

Yes, although you don't need to do the setup on iOS if you only want
alerts on the Mac. You can do the same setup on Mac OS by simply adding
birthdays to your Contact application and making sure the Calendar
application preferences have the "Show Birthdays calendar" option
enabled.
