Supposedly, Qualcomm updates the firmware over the Google Play System Update
mechanism (which itself is either over the air or over the net), usually
transparently so (although the updates apparently only happen at reboot).

If all of your phone came from the manufacturer like Apple iPhones do
then you wouldn't have to worry about third party firmware updates from low
quality companies like Qualcomm.
