Does Alpine/Pine support message threading?
Ideally something similar to what you find in Mutt / Thunderbird or
Gmail with messages/conversations grouped together with a reasonable
degree of intelligence.
A quick web search didn't show up anything obvious.

A colleague just pointed this out in pine.
$ to chose type of sort and H for threaded view.

In addition to all that has been said, the OP may want to press M S C
and configure the value
of "Threading Display Style", the "Threading Index Style" and a few
other options related to threading.
"$H" is nice but it does not do all that is wanted.
This doesn't seem to be happening on PC-Alpine here. Does it only
apply to the inbox? (If so, why?). I can see threading on other
folders but no indenting. How does Alpine identify a thread?

Ok, I can see threading happening if I look closely, I guess I was
missing the very few threaded 'conversations' in a mass of
individual posts, sorry!
Any way of including the replies to complete the conversation, Gmail
style?

I don't know what "gmail style" means.
I find that all of the messages in a conversation, including my
replies, within a folder are properly threaded. I use the S (save)
command to move all of the messages into the same folder so that they
can be threaded.
Sometimes I correspond with people who either
a) send replies which do not include the proper headers so that pine
can thread them. This could happen if their mail client is
incompetent. Or it could happen if the user creates a new message
instead of using the reply function, ie. the user is incompetent.
b) Use the reply command (instead of new and typing in my e-mail
address) to start a new conversation (ie, the user is incompetent).
Either of these cause mis-threaded messages.
