Looking for WYSIWYG form builder

I need to create a WYSIWYG database entry-form builder.
I would like to present the user with a "generic" table form
and allow them to move the widgets around the screen (using
drag-and-drop). Someone suggested I look into using
the "grid".

I'm not looking for a finished product necessarily, just some
example code would be nice.
