Now, I know there's only one listing in the address book, and I checked the
mail directory and there's no duplicate mail file. Any thoughts on how to
clear this up?

Find out the name that generated the "User not Unique" error, remember
that you can get a conflict in first name, last name or shortname.
An Example: I had a slient who had a user named Stan and he could get
messages addressed to "Stan@domainname", this worked until they hired
Susan Tan, whoose shortname was (by default) STan. (we fixed that
particulat problem by changing Susan's short name to SLTan).
