There are some templates at www.oasiscd.com

Probably other CD duplication companies have templates as well.

Make them in Photoshop (use mm as your measure) and carefully measure
everything. Then you'll know how it works.

Try buying a Fellowe's 'Neato' packet which comes w/ the software for making
covers. It's very, very easy to use.
