Anyone have any chilling horror stories?

Apologies for the flippant response, but I couldn't resist, given the
subject line.
1. I admitted that I am powerless over the telephone, and that my life
had become unmanageable.
2. I came to believe that a Power greater than myself (The Phone Company,
of course!) could restore me to sanity.
... You get the idea.
The closest I have to an on-topic response is that at one point I was
working in a small call center for Xerox service, and we were idled for
several hours because a power failure knocked out our ACD (automated
call distribution) unit. We had contingencies for computer downtime,
but not for losing the phone system.

mmm...you are looking for incidents like the Japanese telecom
company recently paying back millions to it's customers due to
an error in their bill.
I suggest you take a look at Telcomine (www.infozech.com/telcomine.htm)
the Telecom newsletter. And their current issue was focussed on Billing. Or
reach them (se...@infozech.com) they'll be able to provide you with a lot
of details. But I guess you might need to give some credit to them as well!

Our outfit runs a radar site in the middle of the New Mexico desert.
Until August of 1997, we used OPEN WIRE CARRIER (bet that brings
shudders on most of you techies out there). We used some radio based
units but the sound quality was HORRIBLE and could not do data above
1200 baud, and that was stretching it.
We finally convinced the local telco (GTE) to put in a spread spectrum
digital T1 microwave circuit to the nearby mountain (35 miles to the
south) and that has since fixed the problem.
But for the last year or so while we had the open wire carrier to
carry 9.6k baud data, it was constantly going in and out. Combination
of several things - the wires themselves were slapping against each
other in the wind, the VACUUM TUBE based equipment kept going out, etc
etc.
I can go on and on, but I think you get the idea
