MLPS or WAN Ethernet Providers for Small Company with 4 Branches?

Hi, I need to know what telecom companies in Wisconsin and Illinois
offer scalable WAN services. HQ would need to start at around 3 Mbps
(bundled=data/voice/internet) and 2 branches would need 1 Mbps and 2
other branches would need 768 kpbs. If the economy picks up, each of
these speeds could double in the next 2 years. We would be looking for
managed service where the routers etc are leased and managed.
Firewalling would be either my company's responsibility or we would
consider adding that to the bundled service. So who should I be sending
a RFP to? We are looking to keep total monthly costs to around $3,000
and require carrier grade up-time. Any help appreciated. TIA
