I am generally familiar with the operation of Diffserv but am unclear
about the way in which routers in the network can actually provide a
guarantee that there is sufficient bandwidth to deliver a given
quality of service to a given traffic flow.
For example, at the entry point into the Diffserv network, the ingress
router marks the DSCP for a Premium service for, say, voice. While
Diffserv routers will give this traffic preference over any other type
of traffic, how can we guarantee that the routers on the route the
traffic actually takes, have enough resources to deal with all Premium
traffic.
Is it that the network needs to ensure that correct ingress policing
is done, or what?

Generally, there is no guarantee on any of the qos class in Diffserv
Network. RSVP support guaranteed service, and that is integrated service.
