ERROR: Abnormal termination, Error Code: C0000005 ACCESS_VIOLATION

I built a form using Oracle Forms 6i on a Windows XP machine. Now I am
trying to compile and run this form on a Windows 2000 machine. Both
are running on the same database.

Problem: The form compiles and runs on the Windows XP Machine with no
errors. In the Windows 2000 case, the form compiles without a problem
but fails to run. It attempts to run the form but immediately closes
and generates the following error file:

have you had a look at

http://otn.oracle.com/software/products/forms/content.html

and/or installed the patches from metalink
(patch 14 and perhaps also the packport patch 6.0.8.24 ?)
