E_US1194 duplicate key on INSERT detected
I can not find "US" in my Ingress manuals.
Anyone have any suggestions as to where to look to find out what
this error is really trying to tell me ?

I am a little rusty but I'll give it a try ....
I think you are trying to insert a value into a unique key column that already
has that value in it. In other words, table foo has column foo_1 that is
specified to contain only unique, non-duplicate key values. This means that
a value may appear only once in that column for that table. So if the value
1 is already in the column than trying to insert 1 again will generate this
error.

US stands for user error, meaning the user caused the error by some invalid
activity like trying to insert a duplicate key.

Should be in your error codes manual. A large, white, bland looking 3 ring
binder about 3 inches thick. Also, if you look in the DBA guides they list
error code classes there in one of the Appendices.
