I have created a guide line sheet of the contracts that require nursing with the type of visit and the frequency of how often we are required to do these visits.

Please use this to guide you, to ensure that we are doing the right visit.

Please Note: When creating a 485 the frequency of nursing has to match this sheet, for Example for a Elant Choice case the nursing Frequency should include: > SN TO SUPERVISE PCA EVERY 3 MONTHS, EVERY 6 MONTHS FOR RE-ASSESSMENT OF HEALTH STATUS, HEALTH PROMOTION, DIAGNOSIS TEACHING, DIET AND APPROPRIATENESS OF PLAN OF CARE. FOLLOW UP AS NEEDED X 180 DAYS.

PCA - x WEEKLY FOR - HOURS X 180 DAYS (The above wording is not an example for clinical terms, this is just an example for the frequency.)
