Survey question using checkbox and hidden question to get different score depending on the number of choices made

I have survey questions each framed  with format as below.  When  selection is none  the  score is  be 1, one  selection out 4, the e score is 2,  and so is others as  appear on the table below.  I have been trying to represent  this information on Cammcare survey using  checkbox and the hidden question to get this result but only get the  score of 1 and others 0 using nested ifs.   Any support /advice  on how to go about this is much appreciated.

You can use the count-selected function to accomplish this task along with if() statements.
