PRV 2310B Rendering Provider Specialty Information is now required.
Its syntax is PRV*PE*PXC*xxxxxxxx~
PE and PXC are forced as they will have a code for PXC,
this is also available in the 2420F

Service Facility Location Name 2310D NM1, (address)N3 and (City,State)
N4 and
their application to 2420C.

Add RENTAXONOMY to latest version.1.17L-21
for Service Facility Location Name use
FACNAME, FACADDR1, FACADDR2, FACCITY, FACST, FACZIP
Example 21 has been set up to use these fields and has been modified
to be 5010
