What kind of documents we have to fill in for apply at WUT, and whom we have to send ?

Hi to all, my university office let me completely alone, so i ask to
you for some help...

I need to regularize my Erasmus student application at WUT, i belong
to the faculty of Computer Engineering.

I have found these form on the "Elektroniki i Technik Informacyjnych":

-Bilateral Agreement

-Student Application

-Learning Agreement

-Transcript of Records


I suppose that i need to send the first three, isn't it?
Who knows the fax number or the email at whom i have to send it?

Please help! :)

It is best if you ask the faculty coordinator Mr. Zabołotny, he can be reached at in...@elka.pw.edu.pl or W.Zab...@elka.pw.edu.pl
He usually responds to emails pretty fast so it shouldn't be a problem.
