Hi there, 

Hope everyone is enjoying their holidays. I am currently looking to fill a care coordinator position within one of my teams at Montefiore Medical Center in the Bronx. The position is not posted yet and I am looking for someone to fill this role ASAP. So please share with your networks. 

Briefly, I am looking for someone who has experience in the following: 
  • Health Education
  • Patient Management 
  • Coordination between clinical staff and community based organizations 
  • Bilingual, Spanish/English
