Request for a Franklin Co ACA Age&Premium table

Dear Bogleheads,

I'd like our club to fill out the attached spreadsheet with Age & ACA premiums for Franklin County.
Any such information, and the process for collecting it, would allow folks
to create a 1095 & 8889 with realistic estimates of ACA subsidies.
By creating the table and also recording the process any member could
execute the process for themselves and the club could repopulate the table each year.

This calculator returns exactly the information I was looking for.

I recommend anyone considering ACA subsidies use the calculator to determine their subsidies.

For those on ACA I would also recommend reading and subscribing to Harry Sit's blog The Finance Buff.    Harry writes a lot on the nuts and bolts of personal finance but he is especially well informed on the Affordable Act.  He covers all aspects of the program from how to sign up, to the mathematics of subsidies, to reconciling one's PTC credits on your tax returns.
