Subject: Request to Share IRB-Approved Survey on Healthcare Experiences During & After Incarceration
To: in...@cure-sort.org <in...@cure-sort.org>

Dear CURE-SORT,

I hope this message finds you well. My name is Mia Hong, and I am a research intern at the Southwest ADA Center and Independent Living Research Utilization (ILRU) at TIRR Memorial Hermann in Houston, Texas.

We are conducting an anonymous survey exploring how formerly incarcerated individuals with disabilities or chronic health conditions experience healthcare after release — particularly how those experiences shape trust in medical systems.

Participants must:
  • Be age 18 or older
  • Live in the United States
  • Have a history of incarceration (but not be currently incarcerated)
  • Identify as having a disability or chronic health condition

The survey takes approximately 10-15 minutes. It is uncompensated, but the results may help improve how healthcare systems support people with disabilities after incarceration.

I’m reaching out to ask if your organization would be open to sharing the survey — whether through a newsletter, email list, or other community channels. The IRB protocol number is HSC-SPH-25-0778, and I’m happy to provide additional information if helpful. Here is the link to the survey: https://miahong290.questionpro.com/t/AdSNHZ9Agc. I have also attached a flyer you may use with distribution.

Thank you for your consideration, and please don’t hesitate to reach out with any questions.
