CPR Class Sign Up Form

Hello all,

As your seasons start to ramp up, don't forget about player safety! In an effort to improve the safety of players under our purview, Altitude Ultimate is excited to offer FREE CPR training to coaches. 

Please fill out this form to attend a training session.

Estimate 2-3 hours for the training. 

Location: The trainings will be held at - Christ the Servant Lutheran Church - 506 Via Appia Way, Louisville, CO 80027.

A maximum of 6 people can attend a single training, so don't wait to sign up, it will be first come first served style. If we fill all slots, we'll look to host some more training sessions later in the Spring as well.

Fellow coaches,

I'd like to encourage everyone to take advantage of the CPR training that AU is hosting!  The initiative to train youth coaches was started in partnership with AU by the parents of Silas Wright.  During the middle school state tournament 4 years ago, 14 year old Silas collapsed in the middle of a point.  He stopped breathing and had no pulse.  There were 3 medical professionals on the sideline that knew how to provide CPR and were able to revive him until EMS arrived. Tragically, Silas had another episode 4 months later and passed away.  His family was grateful for the extra 4 months they got with their son, and they helped to start this initiative in the hopes that it could help someone in the future.
