Entire project reformatted

The entire project on master has been reformatted using clang-format. The format used is in .clang-format.

It's not essential everything is in this format, this was largely to modernize the formatting. I may periodically reformat, you can explicitly turn formatting off with

// clang-format off

and turn it on with

// clang-format on
