I think I would also mention that if they don't want to use either of these forms, they can still create their own. And, if you want to include the statutory reference requiring the audit and appeals policy, it's § 29-11-103(7)(f), C.R.S.

Perhaps:

- From:  Prepaid and Collections Committee of the Colorado 9-1-1 Task Force

- Statute instead of Statue

- Originating Service Providers instead of Telephone Service Providers

- Adjust the first sentence to focus on requirement per statute without connotation of a request

     - If desired a Governing Body may make this a requirement, while a request implies the OSP may decline to comply

     - E.g., "Per State statute, each Governing Body may require Originating Service Providers to use a form provided by the Governing Body

- Adjust the last sentence to "...adopted by resolution or ordinance as applicable, as outlined...", because Governing Bodies which are city councils would do this by ordinance
