What are current CNLs doing to decrease patient falls?

It’s a myriad of elements combined to promote safer and prevent injury:
1. Hourly rounding focusing on Morse Fall Score >45 and the 6 P’s.
2. Monitor call light usage and reason for use and need.
3. Maintain Fall injury prevention measures which include bed in low position, limit side rail use ((LTC), fall matt use aside the bed.
4. Recognize falls related to behavioral issues.
5. Place residents in the same areas, a staff member can monitor those residents and position themselves for ease of visualization.
6. 1:1 for safety to prevent injury for residents who might be confused.

In addition to what was already mentioned. Gait belts hanging in all rooms closest to the bathroom and used on all patients, regardless of score.
Bed and chair alarms activated at all times
Ambulation 4 times a day, not including therapy.
Up for all meals

Hi all,
Just a note to remember to this is in alignment with the Behavioral Health National Patient Safety Goals for ligature risk, that this item is easily removable or in a locked cabinet. This could be problematic if found on survey in a facility where a patient is on a 1:1 coverage for any BH concern.
