Thanks for uploading the form Ajinkya!
You need to get 1 Test and 1 Vaccination done:

1. PPD skin test: They Inject an antigen in the upper part of the skin
and check the diameter 48 hours later. In case the test comes
positive , you need to take an X Ray and attach the report.

2.MMR Vaccine : Most of us have taken MMR vaccine when we were
babies , around 15 months old. In that case we need to take a Booster
Dose (2nd MMR dose) now and mention the dates. If you have not taken
MMR vaccine anytime , then you need to go in for 2 vaccinations
keeping a months gap between the doses.

In case you dont have sufficient time , get a dose now and the 2nd one
once you reach CMU. Vaccinations are free in CMU.
