Broadband connection Requirement details

Dear all CMs,

Please complete this. Those who need the broadband connection in the CFL office should get the 3 Nos. of a quotation from 3 different vendors. Those who do not require a Broadband facility should not fill this but have to write NA. 

For reference, I have attached a quotation.
