Request for review: two-factor auth

Howdy folks!

I've put up a PR that adds optional two-factor authentication to
Clojars via TOTP. Since this is a major change to authentication, I
would like some other folks to take a look at it before I release it.
If it looks good, I'd like to get it out in the next week or so.
