What steps will reproduce the problem?
1. Move mouse on the word
2. Press Shift + Alt
3. Click on Add new word
4. New word is added to database and highlighted

What is the expected output? What do you see instead?
The floating window is displayed. But I cannot see it any where on my
screen.

What version of the product are you using? On what operating system?
Version: 2.5b2
I use Firefox 3.6.9 + Thunderbird 3.0 theme
OS: Win 7 Professional

Please provide any additional information below.
I also cannot remove the added word from database.
