I just about hit a lady today the timing an angles were the perfect storm for not seeing her start to cross the road until I started to make the left hand turn luckily I was driving slow enough and slammed on the breaks but it was way to close for comfort.

The car braked and ran into my shopping , which took the force which was around my knee. But many things ran through my mind. Had I been a kid, I might not been see at all; had the lady driver not seen me when she did, I could have been knocked into the main road and been hit be another car. I swore out loud out of surprise & shock, she waved her hand in apology. You must pass a vision test when you apply for a driver license or to renew your license. The test must show that you have visual acuity of at least 20/40 (based on the Snellen Visual Acuity Scale) in either or both eyes, with or without corrective lenses.

When you renew or replace your DMV document at an office, you can take a vision test there or prove that you have met our vision requirements by providing a Vision Test Report (PDF) (MV-619) completed by a qualified eye care provider. 2

If you want to remove a CORRECTIVE LENS restriction before renewing your driver license, you must obtain proof that you do not need corrective lenses and order a replacement document. Proof can be submitted to DMV

If the Vision Test Report (MV-619) has a print date of 9/03 or earlier, the form is valid for 6 months after the date of the eye test. If the form has a print date of 3/04 or later, the vision care professional marks a box on the form to indicate if the form is valid for 6 months or for 12 months after the date of the eye test.

To renew your driver license online, a vision test provider must enter your test results in the DMV Online Vision Registry or complete a Vision Test Report (MV-619) so you can enter information from the form.

If we have determined that you need this report, it will be listed on the back of your license asa restriction and the form will be mailed to you a month before you need to submit thecompleted form back to us. Your license may also list additional driving-related restrictions (i.e. no nighttime driving, no interstate driving, no manual transmission, etc.).
