Find out about general public access to military records

Recent military service and medical records are not online. However, most veterans and their next of kin can obtain free copies of their DD Form 214 (Report of Separation) and the following military service records any of the ways listed below.

There is a fee for records that are considered "Archival," which depends on the discharge date. If the request is made 62 years after the service member's separation from the military, the records are now open to the public and subject to the public fee schedule (44 USC 2116c and 44 USC 2307). This is a rolling date, the current year minus 62 years. Learn more.

Response times from NPRC vary depending on the complexity of your request, the availability of the records, and our workload. We work actively to respond to each request in a timely fashion, keep in mind we receive approximately 4,000 - 5,000 requests per day.

Records are accessioned into the National Archives, and become archival, 62 years after the service member's separation from the military. This is a rolling date, the current year minus 62 years. See more information on records older than 62 years.

Archival records are open to the public and can be ordered online for a copying fee. See Access to Military Records by the General Public for more details.

You can mail or fax your signed and dated request to the National Archives' National Personnel Record Center (NPRC). Be sure to use the address specified (either in the instructions on the SF-180 or in our online system, eVetRecs). Most, but not all records, are stored at the NPRC. Special Note on Contacting by Email: Requests for military personnel records or information from them cannot be accepted by email at this time. The Privacy Act of 1974 (5 U.S.C. 552a) and Department of Defense directives require a written request, signed and dated, to access information from military personnel records. Our email address should only be used only to request general information (hours of operations, procedures and forms) or to submit compliments, complaints, or concerns.

NOTE: If you send messages using WebTV or a free-email service, you will not receive our response if your mailbox is full. Messages sent to full mailboxes are returned to us as "undeliverable." You may wish to include your mailing address in your message so that we may respond via the U.S. Postal Service.

Once you have successfully paid for your record, you will be prompted to view and print your record. Once you have closed the shopping cart confirmation page, you will need to make another purchase if you wish to view your record.

Online requests only allow for viewing online and printing to your printer. If you would like a printout mailed to you, please complete the INF 1125 form. Mail the completed form and the fee to the DMV Headquarters address displayed on the form.

Alert: To respond to your Freedom of Information Act (FOIA) or Privacy Act (PA) request as quickly as possible, we will stop accepting requests by fax or email on Dec. 16, 2022. We strongly encourage you to use our online FOIA request and response service.

Step 4: Make your FOIA request online to avoid mailing, paper processing, and returned mail that could make it take much longer to process your request. You can also use this service to receive records online, if you already submitted a request by mail. See the chart below.

If you prefer to make your request by mail, download, complete, print, and sign Form G-639, Freedom of Information/Privacy Act Request (PDF, 436.26 KB). Form G-639 ensures we receive the correct information to fulfill your request if you make it by mail.

You may draft and mail a request for records that meets statutory FOIA procedures without using Form G-639. If you do this, USCIS cannot process your request as efficiently as possible. If you mail your request, you may be more likely to omit some information we need to fulfill your request, which could delay you from receiving the records you requested.

However you make your request, be as specific as possible about which documents you need. We can respond to requests for specific documents faster than for large set of records, such as entire A-Files.

Through this service, Texas Driver License Holders may request their own Driver Record online andprint the Record at the end of a successful order. You may also choose to receive the Driver Record by email. You may not request to receive your Driver Record through the mail.

If you moved during your background investigation, contact your security officer, human resources representative, or facility security officer. They can verify the correct information and communicate the update to us at DCSA. We only need updated contact information if your background investigation is open.

Access to criminal history summary records maintained by the DOJ is restricted by law to legitimate law enforcement purposes and authorized applicant agencies. However, individuals have the right to request a copy of their own criminal history record from the Department to review for accuracy and completeness. Requests from third parties are not authorized and will not be processed.

Print out the Application to Obtain copy of State Summary Criminal History Record (Form BCIA 8705, pdf) and follow the instructions on the form. A Spanish version of this form is also available (Form BCIA 8705S, pdf). Please contact your local law enforcement agency for fingerprinting services. PLEASE NOTE: Your fingerprint card must contain your full name, date of birth, sex and return mailing address. If you are having difficulty in obtaining a blank fingerprint card, please contact the Record Review Unit at (916) 227-3849.

Once you have received your Record Review response, if you want to challenge the contents of your criminal record, you must complete the "Claim of Alleged Inaccuracy or Incompleteness" (form BCIA 8706), which will be included with your Record Review response if there is criminal information on your record. Mail the completed form, along with a copy of your criminal history record, to the address indicated on the form. Your challenge must specifically state the basis for the claim of inaccuracy or incompleteness and include any available proof or corroboration to substantiate your claim.

You can request an immunization record for yourself or your child from the Colorado Immunization Information System (CIIS). Immunization records from CIIS may not be complete. CIIS immunization records include what has been reported to and entered into CIIS.

Patients and parents/guardians of minors can also request their record by completing and submitting a form. Incomplete forms (e.g., forms without proper ID or forms that are not notarized) will not be accepted. Patient immunization records can be securely transmitted to the requester via fax or secure email.

If you're a super admin, you can set up a large export warning notification to know when users are taking record data out of your account. Each super admin can customize their notification to their own needs.

If the records you export have associated records, each associated object will have its own column within the file. Depending on the object you're exporting, you can export information about associated contacts, companies, deals, tickets, custom object records, line items, subscriptions, or payments.

The file format XLS has a limit of 65,536 rows, so if you've chosen that file type and exported more than 65,536 records, records beyond that number won't appear in the file. You can repeat the export and select to export the records in an XLSX or CSV file instead.

If you notice discrepancies on your driving record, you will need to contact the listed court for tickets and the listed police department for accidents. Corrections will then be sent to the Michigan Department of State to update your record. If you need to discuss other details of your record, call our Driver Records Activity Unit at 517-636-6406.

The link for your electronic driver record is only accessible for 7 days. During the seven-day period, you may view your electronic driver record as many times as you need to by logging in and choosing the option to View Transaction History. We recommend you save your documents to your computer or device so that you may conveniently view the record at any time. If you were unable to access your driving record in the time allotted, you may apply for a refund.

If you are purchasing your driving record for your own reference, you may not need a certified record.

If you are purchasing a copy of your record for a court case, or for an employer, check with the person requesting the record to verify what type of record is needed and if it needs to be certified.

NOTE: All driving records purchased in Secretary of State offices are certified records.
