Table of Contents Left Column

Hello!

I've tried various options to correct this (even using --no-toc) to
eliminate it, but it consistently reappears and is smushed (for lack
of a better term) into a column on the left side of the page that's
~two words wide. Any help you have would be most appreciated.

chm2pdf uses a program called "htmldoc" for the PDF conversion. htmldoc is not perfect and it seems that it gets confused by some table in the HTML file(s) that are part of your CHM...

If no combination of options works, you may be out of luck (it would mean that htmldoc cannot convert that bunch of HTMLs correctly, for whatever reason...).
