Scarcity on Battle.net and scarcity on single player do not present themselves in the same way. If you find a Ber rune on single player, congratulations; you just found a Ber rune! If you find a Ber rune on BNet, then you may have just found a Ber rune, or a Lo, or a Sur, or a Jah, or whatever a Ber rune is worth to a particular player in a trade. Obviously scarcity still exists on BNet because only rarities will yield rarities, but do you see the vast discrepancies in how scarcity presents itself?
As a follow up to my can I still dupe the hell out of Diablo 2 runes/gems, which I intended more for the single player fun factor, I am curious to know if there is a well developed strategy for obtaining gems and runes without duping?
When first killing Countess on Hell, the highest rune that can drop is Lo. Every time after that, the highest rune she can drop is Ist. She'll pretty much drop runes every time you kill her (very low chance of getting no runes). Baal on the other hand can drop all of the highest runes, including Zod (which I believe is a rune that can only be dropped in Act 5 Hell). From Kotsu's comment
The monster with the highest probability of dropping runes is the countess in Act 1 (Forgotten Tower). I think she even drops at least one rune every time. AFAIK Andariel and Nilathak also have a rather high chance of dropping a rune, but not even close to the Countess. Nilathak can drop much higher runes though.
Oh, btw, for gems, see option 1 up there. On SP, you can engage in Gem Shrine Farming. Basically, you run and rerun The Stony Field, The Dark Wood, and The Black Marsh looking for "Gem" Shrines. Carry some flawless gems with you but no other, lower gems, and when you find the gem shrine, hitting it will upgrade one of the gems in your inventory/backpack to a perfect gem. You can do this on bnet, but since the maps change with every new game, it's really not efficient.

As players advance through the game, the possibility of obtaining higher level Runes increases. There are a few exceptions to this, but in general, the table below outlines the Runes that can begin to drop in each Act and difficulty level. Note that the list is cumulative, meaning that if Zod can drop, so can all other Runes (this also does not include terror zones which have increased monster ilvls during the period):
The Countess is a demon Superunique monster, and can be found in Act1 at cellar level 5 of the Forgotten Tower, which is in Black Marsh. She is the only monster in the game with a uniquely high chance to drop runes. On normal difficulty Ral Rune, on nightmare Ko Rune, and on hell Lo rune is the highest she can drop. However, the very high drop rate only affects runes up to Io Rune at nightmare and Ist Rune at hell difficulty. As an example, on hell she has a 1:287 chance to drop an Ist rune, while the second highest drop chance of an Ist rune is 1:7789 by killing the Act2 Boss Duriel. By using the rune upgrading horadric cube recipes, The Countess is the best source even for high-tier runes up until Sur.
The Countess's rune drops are calculated in a unique way. First, five rolls are made in her item drop table, each with a 19/67 chance to roll nothing. Next, three rolls are made from her rune drop table, each with a 1/4 chance to roll nothing. If a rune is rolled, it first rolls the highest rune for the corresponding difficulty. If it doesn't roll a rune, a guaranteed El or Eld rune is dropped.

Lower Kurast is an ideal Super Chest farming location because its map always spawns with a hutt containing three Super Chests. Focus on high survivability and mobility when farming Lower Kurast, as there is no need to kill monsters. Stick to higher difficulties (Players 7) for the best chances at obtaining high runes. Going to Players 8 will not increase the likelihood of rune drops when compared to Players 7.
To find Ber, Jah, Cham or Zod runes, doing a lot of grinding is inevitable. Bosses have 10-20x higher chance to drop these runes than regular monsters, from Duriel dropping Ber runes once every 42.945 kills, to Hephasto The Armorer dropping Zod runes once every 192.066 kills.
Normal Cows (Hell Bovines) have an 1:730k chance to drop a Ber rune, and an 1:2560k chance to drop a Zod rune. Despite these low chances, such high runes are still the most likely to be found by farming cows, as about 350 to 450 Hell Bovines appear on the Secret Cow Level. To efficiently farm the Cow Level, your character build of choice should be one that deals a huge amount of area damage, allowing you to cut through Cows quickly. In addition to being the best source for high-tier runes, the Cow Level is also the best source for socketable base items. In conclusion, it is the go-to place if your aim is to acquire high-tier runewords.

Early game what we mean here is the early end game, so you have gone into Hell, your build is functional in hell and you're still using a balancing Act between magic find, kill speed, and survivability, you're not going to have the best in slot gear in any of your slots and you're probably trying to make the decision of the equipment to use as you're trying to farm.
Elite sniping is the second of the three primary farming strategies you are not prioritizing player settings because we're abusing the fact that the monsters are always going to drop Diablo 2 items. This is typically high magic find build, there are some nuances to that, but on average this is low player settings, high magic finds and most builds can competently start elite sniping in the early game.
The Pit Zerker is the best version of an elite sniper that's actually an endgame focused build, because you need to have stuff like Enigma to be able to do it efficiently. If you want to think about early game options here either a frenzy or double throw barbarian, any sorceress that's trying to elite snipe utilizing fist of and any build which is able to prioritize mobility either on swap with a harmony bow or teleport staff, and you can also consider just farming super uniques that are right off from a waypoint, we are trying to kill monsters, low player settings, high magic find.
